  • FSA

    • With respect to a health FSA, an employee to revoke an election, make a new election, or decrease or increase an existing election.

    • With respect to a dependent care FSA, an employee to revoke an election, make a new election, or decrease or increase an existing election. 

    • Treasury and the IRS also note that in determining the extent to which election changes are permitted and applied employers can limit mid-year elections to amounts not less than already reimbursed.

    • FSA carryover is $610 for 2023.

Medical Insurance

 

HDHP (HSA eligible)

A High Deductible Health Plan combines a Health Savings Account (HSA),traditional medical coverage and a tax-advantaged way to help save for future medical expenses while providing flexibility and discretion over how you use your health care dollars.

PPO Plan

​

A Preferred Provider Organization plan offers a lower deductible for individuals and families along with co-pays for office visits and emergency services.

Health Savings Account (HSA)

​

An HSA provides pre-tax savings for medical, dental and vision expenses with High Deductible Plan only.  Funds roll over from year-to-year. No “use it or lose it” philosophy.  

 

2023 Annual maximum contributions:

$3,850 | Single
$7,750 | Family

Persons age 55+ are entitled to an additional annual catch-up contribution of $1,000 (only available if you choose the High Deductible Plan).

Flexible Spending Accounts​

 

An FSA provides ​​opportunities for pre-tax Heath and Dependent Care spending. The maximum carryover at the end of the year is $610. Any funds remaining are forfeited.

Dental

​

Diagnostic and preventative services are covered at 100%.
Basic services (routine and restorative) are covered at 80%.

Major services (endodontic and periodontal) are paid at 50%.

Maximum dental benefit per year: $1,000 per person

Orthodontia coverage is for dependent children only and has a lifetime Max of $750.

Vision ​

VSP provides employees and their families with an affordable eyecare plan including:  

​

$10 eye exam co-pay

$130 frames allowance (every 24 months)

$130 contact lens allowance

Additional savings available for sunglasses, Laser vision correction and more!

To Participate in the Prescription Safety Glasses Plan, employees must be enrolled in CVA vision insurance.

Safety Vision

​

Short Term Disability

Short Term Disability is voluntary and 100% paid by employee. Coverage starts after 14 days of illness or injury and pays for a maximum of 24 weeks. Full-time status is required to participate. 

​

​

​

Long Term Disability

​

Long Term Disability is 100% Paid by CVA with no cost to employees.Coverage starts after 26 weeks of illness or injury and pays 60% of wages with max of $8,500/month (subject to base pay)Full-time status is required to participate.

​

Life Insurance​

Basic term life insurance is paid by CVA at 100% with no cost to employee.

Coverage is provided at 2.5 times your annual salary for employees, $10,000 for spouse and $5,000 for each dependent child, live birth to age 26. This policy also includes AD&D that doubles the volume in the event of an accidental death.

​

Employees may also elect supplemental term life insurance on themselves, their spouse and/or dependent children.

​

Voluntary Benefits

Accident 

Accident insurance will help protect you and your covered dependents from unexpected accidents and injuries off the job. Our plan will pay benefits directly to you when you need it most to help pay for out of pocket expenses.

Critical Illness

​

Critical Illness insurance will pay a lump sum benefit directly to you if you are diagnosed with a critical illness or cancer.  The money can be used for medical bills, treatment, income, or even your everyday bills and expenses.

​

Universal Life insurance is uniquely designed to match the needs of employees throughout their lifetime and provides permanent coverage to supplement the basic term life insurance.  With locked in rates and cash value, this coverage offers more flexibility compared to traditional life insurance.  A variety of options are available to accommodate your needs and budget.
