• Basic services (routine and restorative) are covered at 80%

• Major services (endodontic and periodontal) are paid at 50%

• Calendar year deductible

  • Individual: $50​

  • Family: $150

• Max dental benefit per calendar year: $1,000 per person

• Orthodontia coverage is for dependent children only and has a lifetime maximum of $750
